Substance Use Evaluations

All clients undergo a comprehensive assessment, not just of the presenting issue, but of all possible underlying issues or co-occuring disorders unveiling the root cause. This approach vastly increases the chance of sustained recovery and high quality of life because we target in on the issues that substance use  covers up and work towards healthy healing.

DUI Evaluations

Frustrated that you are charged with driving under the influence? Can't believe this is happening to you?   Let us help you get your license back and your life.  We will give you an evaluation to determine your risk level and what level of treatment best suits your needs. At Mindset Transformations you don't have to wait weeks or months to get started with groups , you can be placed in a group that meets your individual needs after you receive the outcome of your evaluation.  We are happy to be on your team and want to be apart of your journey  to reclaiming your life back.

DOT SAP Evaluations

We understand your fears and frustrations right now. Your livelihood is at stake and Mindset Transformations takes that seriously. If you are an employee working under DOT rules and test positive or refuse a DOT drug or alcohol test, the employer must follow specific federal guidelines before allowing you to perform (or be hired to perform) safety-sensitive duties.  Mindset Transformations  provides this service to truck operators, all DOT drivers and workers,  pilots, heavy equipment operators, pipeline, and HAZMAT workers. Please feel free to call us to discuss your specific situation.   After speaking with you we will schedule an in-person appointment within  24 to 48 hours to begin your assessment process and give you specific steps you need to return to duty.

Level 0.5
Early Intervention: Adults and Adolescents

This level of treatment is designed for those who are at known risk for developing a substance use disorder. It’s also for people who have shown signs and symptoms of a substance use disorder but do not meet diagnosable criteria for it.

At this level, treatment is directed toward the risk factors for developing a substance use disorder such as:

  • Mental Health decline

  • Trauma and abuse

  • Aggressive childhood behavior

  • Lack of parental supervision

  • Poor social relationships

  • High degree of substance availability

  • Drug experimentation

Mindset Transformations provides early intervention services within the community, schools, and at our agency to assit in healing these issues and providing guidance to increase protective factors. 

Level 1 and 2
Treatment Services: Adult and Adolescents

Treatment  services includes Basic Outpatient and Intensive Outpatient Services.

 At this level of care, clients maintain their daily lives at home but meet regularly with their certified substance abuse counselor and treatment team for groups, family sessions, and holistic services.

Family and Children's  Program

We understand the pain that comes with loving a substance abuser and the dynamic around it.  Your feelings are like a rollercoaster and you fluctuate between loving the substance user, having hope they will change, and sometimes total despair. We know you have experienced many fears, tears, and sleepless nights.  We want you to know that we are here for you too during this journey towards healing.

We believe that the strongest chance of healing occurs when we treat the family as a whole unit.  We offer weekly family and children groups.  We provide family meal time, activities, discussion and holistic experiences to help all family members, grandparents, spouses, caregivers, and especially children understand and find healing peace  with the foundation of the three c's:

  • You did not cause it

  • You can not control it

  • You can not cure it

In the midst of it all, we want you to know that you are not alone, you are ok and can heal too.

PHP Outpatient Day Treatment Program

The Partial Hospitalization/Day Treatment Program runs five days per week, Monday-Friday, from 9am-5:30 pm for individual and group counseling sessions.

 

Treating a substance abuse problem is often a multilayered process. As clients progress from the depths of addiction into a newer and healthier lifestyle, they can still enjoy the freedom of living at home and remain connected to their support groups and treatment networks at the same time.

Partial hospitalization/Day treatment offers high-quality, vital care to break the dependence on drugs and alcohol and to address the underlying causes behind addiction without the need for 24-hour supervision and monitoring that residential treatment provides.

People who attend partial hospitalization programs or intensive outpatient programs have often shown enough improvement that they are strong enough to leave treatment at night and stay healthy. Partial hospitalization is also sometimes used in cases where individuals have family commitments — such as caring for children or elderly family members — that they can’t leave to partake in residential care.

Mindset Transformations partial hospitalization treatment is very structured and concentrated. If a client does not show expected signs of improvement, if attendance at partial hospitalization treatment sessions is negligent, or if a relapse occurs, the client  may be asked (or required) to be referred to a residential treatment facility until their condition improves.

​

Mindset Transformations, Outpatient Partial Hospitalization Day Treatment Program is an ideal step down from Detox or Residential Treatment as clients put in practice what they learned while in inpatient treatment but now can have the freedom of being at home. Like any good freedom, this is also a responsibility, because clients will have to apply and adapt what they learned in treatment sessions to real-world environments, such as:​

 

  • Resuming academic/professional life

 

  • Interacting with healthy friends and family members

 

  • Resisting the temptation to use drugs or alcohol when they are on their own

 

 While there is more freedom in an outpatient setting, partial hospitalization will offer the same dedicated treatment and professional staff that residential treatment offers. That means that support groups, as well as individual, therapeutic groups, and family therapy sessions, will still be a part of the treatment regimen on the healing journey.
